Se realizó un estudio de cohortes retrospectivo multinacional (proyecto INCREMENT) desde el 1 de enero de 2004 hasta el 31 de diciembre de 2013. Se incluyeron pacientes con ISB monomicrobianas clínicamente relevantes debidas a ECP y se asignaron aleatoriamente a una cohorte de derivación (CD) o a una cohorte de validación (CV). Las variables se evaluaron el día en que se dispuso de los resultados de susceptibilidad, y la puntuación predictiva se desarrolló mediante regresión logística jerárquica. El DC y el CV incluyeron 314 y 154 pacientes, respectivamente. El modelo de regresión logística final del DC incluyó las siguientes variables: sepsis grave o shock en el momento de la presentación (5 puntos); puntuación Pitt igual o superior a 6 (4 puntos); índice de comorbilidad de Charlson igual o superior a 2 (3 puntos); fuente de la ISB distinta de las vías urinarias o biliares (3 puntos); tratamiento empírico inadecuado y tratamiento dirigido precoz inadecuado (2 puntos). La puntuación mostró un área bajo la curva receiver operating characteristic de 0,80 (IC 95%, 0,74-0,85) en el CD y de 0,80 (IC 95%, 0,73-0,88) en el CV. Los resultados para la mortalidad por cualquier causa a los 30 días fueron similares.
